Liability Waiver & Release — GBA 2026 Summer Program

I, the undersigned parent or legal guardian, grant permission for my child to participate in the GameBreakers Academy 2026 Summer Program taking place in Carson, California from June 16 – July 23, 2026.

I acknowledge that participation in athletic and physical activities involves inherent risks. In consideration of my child's participation, I hereby release GameBreakers Academy, its officers, directors, staff, volunteers, and agents from any and all claims, damages, or liability arising from participation in this program.

Medical Authorization: I authorize GBA staff to obtain emergency medical treatment for my child if I cannot be reached in a timely manner. I confirm that all medical, allergy, and medication information provided in this registration is accurate and complete to the best of my knowledge. I agree to notify GameBreakers Academy immediately of any changes to my child's medical status prior to the program start date. I understand that GBA staff are not medical professionals and that any information shared will be used solely for my child's safety.

I grant GameBreakers Academy permission to photograph and/or video my child during program activities, and to use such media for educational, social media, and promotional purposes.

Refund Policy: Cancellations requested before June 9, 2026 (one week prior to program start) are eligible for a full refund. No refunds will be issued after June 9, 2026. Participants on the weekly plan who cancel after Week 1 begins are not eligible for a refund on weeks already billed.
